Vengeance doesn't hit when on top of enemy

I’m running into an issue where when I use a skill that puts me close to “on-top” of an enemy, Vengeance skill doesn’t seem to hit (Using Lunge into Vengeance, same issue with Warpath if you stop Warpath too close to the target enemy) Issue can be seen here:
