Some unique concepts came to my mind and I wanted to share them with you. Hope it will give some cool ideas to EHG !
One handed weapon
Copies all stats and affixes from the other one-hand weapon you wield.
All damage this weapon should deal are converted into void damage.
Has 20% chance to apply Time Rot on hit.
40% increased void damage.
One handed - offhand relic
A percentage of incoming damage equivalent to your %missing health are instead dealt to your mana. (at 100% hp nothing happens, at 1% hp 99% of the damage are dealt to mana first).
X% of your spell damage are leeched as mana.
Attackers are drained by X mana when they hit you while you are under X% hp.
You get X% of your mana as increased spell damage.
+1 potion slot
X% increased chance to drop potion from enemies.
When your hp goes below X%, you instantly use a potion. If you don’t have potion, gets X% movement speed buff for X seconds instead. (X sec cooldown)
+5 Vitality and +5 Attunement
(Skin suggestion : a syringe full of necrotic/green fluid)
You can no longer use attack skills.
All your spell damage are converted into poison damage. You can’t deal any other damage type.
Ignite and shock chance are converted into poison chance.
X% increased poison damage.
(Skin suggestion : a Huge badass scale :p)
X% chance to block
+X block protection
+X cold resistance
X% or your cold resistance are dealt as cold damage to attackers.
X% to chill with attacks and with the previous retaliation effect.
Plot of the assassin
Intelligence no longer grants ward retention and instead grants crit chance and crit multiplier.
Skills that scale with Dexterity now scale with Intelligence.
Skills can’t scale with dexterity anymore but Dexterity now gives X% more dodge chance.
When you critically hit an enemy, you get X Intelligence and dexterity, stacks up to X times. Last X seconds.

Skills that scale with Strength now scale with Intelligence
Skills that scale with Intelligence now scale with Dexterity
Your attributes (int, str, dex) are increased by X%
You can’t equip any weapon nor off-hands.
Double the affixes of your Chest armor.
You can use attack skills with your bare hands.
+X% increased health regen
Shackles of despair
Act as double one-hand weapons and works with dual wield effects.
You have X% reduced movement and attack speed.
X added physical damage
Enemies you hit are linked to your shackles. If they go further than X meter from you, you deal them X physical damage and put X bleed stacks on them.
If you have X enemies linked you at the same time, you grasp them all to you and after 1 second you throw them away (enough to trigger the previous effect)
You have 0 potion slot
You get X% of the potion heal as health regen.
Walking on a potion charge your inoculator, increasing the heal regen it provides by X% and your movement speed by X%
