So, i was killed by one of the Necrotic mobs that kamikaze when they die and on respawn i had a persistent bleed effect degen on me.
When i died it said after that “You have been slain by a blood thether from.”
Dieing again would keep the debuff. Restarting the game fixes it.
Seems the bug is very easy to reproduce, happens also when you change zones and then de debuff persists untill you leave the game.
